This sounds like it's your router's firmware not playing nicely with SIP traffic over NAT. Since I can't find your model on the dd-wrt website as it's the integrated voip device, try looking for an update to the firmware.

Is your cable modem acting as a modem/router? and double NATting is playing games with you? Do you get a public IP within the router?

It sounds as if your Router is not playing nicely with NAT. Are you using the default firmware? Have you updated your firmware of your router? DD-WRT is available for some versions of your model.
