The $50 is a one time fee to give you all the settings you need to run a hardware ATA with our services. If you want to buy our ATA then it's $79 + $20 FedEx Shipping. Number porting is $19.95.
There are absolutely no hidden fees.
You do mention the 911 fee - that's something that we will be rolling out over the next several months. Its not our choice, rather it's a federally regulated fee that all phone providers must charge (some will outright state it and others will have it in their regular price). None of this money will be going to us though.
When this cost comes out we will not just start billing existing customers. There's still a lot of discussion going around to figure out what will happen regarding existing customers and this new fee.

When the fee is phased in, would it be possible for those who do not need e911 service to opt out of it, thereby avoiding the fee?

UPDATE! I come bearing great news. A meeting here has just finished and the 911 fees will not be passed on to customers. Info will be removed from the site promptly and everyone will get free E911 service from us.
