Using our Granstream 286 from FPL, when we receive calls most of the time we are unable to hear the person calling us and we must call them back or hang up and wait till they call back. The other person can hear my voice as well. Any help would be appreciated.

It sounds as if your Router is not playing nicely with NAT. Are you using the default firmware? Have you updated your firmware of your router? DD-WRT is available for some versions of your model. Check out here http://dd-wrt.com/site/support/router-database for the info.
If your version is supported by DD-WRT I'd suggest flashing it to DD-WRT. Otherwise, download and install the latest Dlink firmware, configure your port fowarding (info in the FAQs), and that should fix your problem.

I have attempted the port forwading you suggested and no luck. And the firmware that I have on my router is the shipping firmware and current for the router. Any other advice???

The default RTP port for that ATA looks to be 5004. Get the router to forward that port to the ATA and see what happens.
