Hello! I have been using Fongo Home for a few years but I'm now planning to cancel it. I have a question here. Do I need to return the equipment (i.e. the adapter) to Fongo? I think I paid for the equipment when I purchased the Home Phone service. Thanks!

No. You own it.
If you want to use the Grandstream ATA with something other than Fongo Home Phone, you'll need to submit a ticket and request to have the ATA removed from Fongo's provisioning server:
https://support.fongo.com/hc/en-us/requests/new.
