I have tried both these numbers from my PAP2T and they went through OK.
I would try the default dial plan suggested by FPL (which is the one I have on my ATA) and test it. You have a few extra things in there that might be interfering with those numbers. I can't see anything obvious, but if you test this dial plan and it works, then you will know where to look.

From you dialplan it looks like 514 394-5555 should not work. Try changing |011xxxxxxxxxxxx.| to |011x.| and dialing 0115143945555. or adding an entry for the 514 area code if that is what you want. Likewise for the 1 888 835-6281 number should not work but you could add an entry for toll free numbers something like |18[05-8][04-8]xxxxxxx| but the available toll frre number are continually expanding so this may change.
