There is a 30 second or so delay, but it does ring on both ends.

It rings fine now, in and out.
I removed the port forwards as well, and it still works.
I'm still planning to replace the d-link with a newer router, that has a bigger range. Is there an N router anyone would recommend?

I hate to revive this thread but I ended up upgrading to a Linksys WRT160N router. All was going well until the router stopped working, and I had to have it replaced by Linksys under warrently. I still have the DI-624 as an access point. I forwarded the ports, but I can't get a dial tone.
