I am searching the forum and Internet. I didn't find any official information about port forwarding.
After I did a search, I found the following but it is not official
5060-5061 UDP for ATA Adaptor
6060-6061 UDP for FPL softphone
13000-13001 UDP ????? <- RTP (I guess) ??????
16xxx-16xxxx UDP ????? no idea
12000 UDP ????? no idea
What is the range for RTP ports and What is the range for SIP protocol (ATA adaptor) and what is the range for SIP protocol (softphone)
I need official information what ports need to be forwarded to router.
People no need to guess around and open an un-necessary ports to reduce the risk of hacking.
Need official infomration from FPL (Port forwarding)

I am not official, but I am experienced. I believe that should be good enough.
You only need 2 ports forwarded, a SIP port, and an RTP port. For a SIP port, you only need port 5060 forwarded. These other ports above are choices you can select if you already have port 5060 assigned to something else, or your carrier blocks port 5060.
As for an RTP port, You should be able to get by with only one port, and it can be any unassigned port, port 12000 is only a suggestion for an RTP port because not much else uses it. You might want to assign ports 12001-12009 also, because some SIP devices seem to want more. I would stick with one RTP port first, and see what happens.
EDIT: for SIP ports, you have quite a choice, from 5060 to 6060. I never used the 13000 range personally. As I said before, you only need one SIP port.

The FAQ stated the softphone only. Is it the same as ATA Adaptor?
So which one is a SIP port (5060 for sure but I don't know 6060 and 6061) and which one is a RTP port.

eg. from my ATA box.
local SIP port: 5060 (default 5060)
local RTP port: 5004 (1024-65535, default 5004)
so you would port forward 5060 for sip and 5004 for RTP.

With a decent router , you should not need to forward any ports. Just putting it out there
