Forgot my login info, please help

Hello administrator, I have a freephoneline number and have it forward to my home number. I usually use browser remember the login info (username and password), but recently my computer crashed and I re-installed my computer, then I realized I dont have any login info now! I tried all I can remember but no successful... Now I only remember my number and the forward is still there, can you help me get my login back?
