Can't call out or receive calls

I can't call out or receive calls from time to time unless I have to reboot my ip-phone. When this problem happend, the SIP status showed "disconnected" and log showed "Destination out of order" before November, 2013. Now when this problem happened, the SIP status still showed as "connected" but when the call came in, the log showed as following.

SIP Status: connected
SIP User Agent: Grandstream GXP2000 1.2.5.3

Any insight?
