I'm at work and can't really do any troubleshooting, but when I call home, I go straight to VM...no ring at all. My wife just called me from my FPL line, so outgoing seems to work. I won't post my whole number in an open forum, but maybe this will help (905) 228-32XX.

Are you using the softphone, or an ATA? What do you have for a router?

Straight to VMail - FPL softphone
Having done a test today with my FPL software, I notice that "NO CONNECTION TO SERVER" pops up at irregular intervals and lasts for varied amounts of time. Same setup that "works" (if you want to call it that) most of the time for both incoming and outgoing calls. Note that I do have problems with losing audio and choppy audio mainly during incoming calls and DTMF hardly works at all. FPL softphone also tends to crash and act unreliably when on call (loss of input), but the call itself goes through. I am not sure how many of my incoming calls come direct to voicemail, and if that is while my software is up and running, or while it is transitioning in and out of connection.
Of course if you are using an ATA, it is a whole different ball game, but there are several here who can offer their expertise to you on that.

I probably should have mentioned that this is a setup that has worked for months. same modem, same Asus (w/Tomato) router, same ATA.
I did a full reset this morning and nothing changed.

OK, I went into Line 1 configuration on my PAP2 and started pushing buttons...wow I just sounded like my wife.
Since I guessed that my issue was with some kind of forwarding, I Changed the following to "No"

My question is: which of these is the culprit? Have I done more damage than good? Is there a guide somewhere to all of these settings?

Is it possible someone typed in *78 on a phone, maybe instead of *98? On my PAP2T *78 will enable the DND (Do not Disturb) feature. You changing all those settings to no may have disabled it. All of those settings you changed to no in mine are set to yes.
I would change them back one by one and see which one breaks your line again. If it is the DND one, then try pressing *79 on your phone to disable DND. Look on the regional tab to find other * numbers that might help.
Or just leave it as is as it seems to be working.

Jake, you're a genius. DND was the issue, *79 was the answer. The thing that confused me was that I read somewhere that when DND was enabled, the incoming caller would get a message saying that the call recipient was unavailable.

I expect setting the DND on your PAP2T just tells the FPL system that you can't accept calls, so it just goes to voicemail. If you have it connected to a PBX then it might be able to be set up to give a different message.
