*NO* In-coming Caller I.D. & Outgoing Name Display

I hoping someone can assist me in troubleshooting some Home Phone Call Features, which have never functioned properly since my service activation 1 month ago.

1) when placing Outbound Calls, my name does not usually display on the incoming phones Caller I.D. ( it has, but not often. )

2) when I recieve Inbound Calls, no Caller I.D. Info is displayed, Phone Display reads " Incoming Call" however, if I'm on a Active Call and I receive another Inbound Call, phone display shows accurate Caller Name and Number info.

Does anyone know how this can be resolved ?
